Everything you need to know on how to be able to work in UK as a caregiver are discussed in the article where you posted your comment. You just need to read the article thoroughly and understand. With regards to how your sister can help you, I think you should ask her because she is already there.

hi i am here in uk as a student for a year now and my visa expires next year…i want to extend my stay or maybe to obtain a work permit but i dont want to take level 4 anymore as i need to pay again and the workplacement i am in now is i think is imposible to give us a work permit but i dont want to wait for them i need a security that i can hold on to for me not to go back in the phils and start all over again…can you giveme an advice on where to go to or what to do? can you suggest any agency that can help with my situation? tanx pls email me…

To qualify for caregiver jobs in the UK, you need to be registered with the GSCC or the General Social Care Council in UK. The GSCC is the regulating body aimed to guarantee a more regulated industry and better protection for those who want to work as caregiver in the UK. You also need to have an employer in the UK who is willing to sponsor you.
